Can the lumbar support be adjusted both up and down and in and out?

There is no lumbar adjustment. I put a small pillow between the back frame and mesh, and I get plenty of lumbar support.

Does this require assembly?

Yes, but I chose this chair in large part because it is so much easier to assemble than most chairs. The chair comes in a giant box because it doesn't come in small pieces, but rather already mostly assembled. I don't have a mechanical bone in my body, and it took me literally about 3 minutes to complete the assembly. It was really that easy. And the chair has proven comfortable and durable.
