Does the tank have any protection in the event someone attempts to drill a hole and attach a towel rack etc to the other side of the wall?

Not that I remember seeing. The frame around the tank is nice and sturdy but I remember the tank being somewhat open. We love ours. It was very difficult finding a plumber who could take on the challenge but once we did and got it installed, we were very happy with it. Because we were working with in such a small space, we had to pack a lot behind that wall (there were electrical wires and outlet boxes, etc. as well as The tank and pipes for the sink) so I would not even attempt to hang anything above my toilet! (Other than something that could stick on the wall.) Anyway, Good luck!

Whats the drill size to fit a drain pipe on a 2x4 frame, without expanding the 2x4 wall frame??

3-1/2" hole saw. That skims the edges of the 2x4 plate. I ran a guard strip on both sides just in case, even though I did the work and was aware when screwing in the sheetrock.
