Which Amazon vendor would be the best one from which order Theralac? Who stores it in a temperature-controlled environment?

Vitacost ships them with a cold pack. They have specials of 20% off frequently (like right now - Sep 11, 2016) promo code GUT20. They also store them properly until shipped, as well.

What are the ingredients (specific details and amounts)? A picture of the label would help?

Lactobacillus acidophilus (10 billion cfu), bifidobacterium lactis (10 billion cfu) lactobacillus rhamnosus (2 billion cfu) lactobacillus paracasei (5 billion cfu), bifidobacterium lactis (3 billion cfu)

There are contradictory answers here, Can you CONFIRM that this ships with thermal protection/icepack? One says it does, the other refutes that.

That is a good question. It basically depends on the seller you are purchasing from. Generally any seller that is "FULFILLED BY AMAZON" will NOT ship with any refrigeration or ice pack because the item is housed and shipped by Amazon warehouses throughout the US. Typically sellers that fulfill the items themselves can make special arrangements for shipping an item because in MOST cases, they are a smaller company that can accommodate the little things, so to speak. This is the broad assumption. The only for sure way to know is to contact the seller prior to making a purchase.
