does this require batteries? Can I extend a wall coaxial cable wirelessly to the cable box so I don't have to run unsightly wire?

No need battery .Extendsion cable is a good idea !

Does this work with MHF4 connector?

sorry it can not .it works with ipex 1 .

I receive my order yesterday no instructions how and where do I mount these to extend my signal do they plug-in to the coax cable or where ?

The antenna can connector to your router directly ,is your router has a SMA FEMALE JACK ?? The cable is a auxiliary tool,it can help you connetcor to the ipx.ufl connector and ebeing a extension cable !
