Can this pan be used on an induction cooktop?

As others have said, T-Fal has some cookware that works with induction and some that does not. I bought this one

and tested it when it arrived. It works well on my induction cook top.

How deep is it?

From the top of the pan without the lid, it is 2 3/4 inches. From the top of the lid rim, it is 3 inches deep.

Does this work well on a ceramic flat top cooktop stove?

If it is a regular smooth top ceramic stove, yes. I've successfully used this pan on mine for years.
